The Survey Period begins on June 6, 2025, at 12:00 p.m. Pacific Time (“PT”) and ends on June 13, 2025, at 12:00 p.m Pacific Time (“PT”), provided it’s not terminated earlier by PurposeMed (the “Survey Period”). Participation in the Survey constitutes your full and unconditional acceptance of, and agreement to be legally bound by, these survey rules (the “Survey Rules”).
1. ELIGIBILITY:
To be eligible to win, a person must: (a) be a of the United States (excluding Florida, New York and Rhode Island), (b) have reached the age of majority in their province or territory of residence, and (c) have been contacted by PurposeMed to participate in the Research Project.
2. HOW TO PARTICIPATE:
No purchase is necessary. Entries that are incomplete, irregular, have been submitted through incorrect means, or do not conform to or satisfy any condition of these Prize Rules shall be disqualified. Entrants acknowledge and accept the decisions of PurposeMed, or those acting on behalf of PurposeMed, as final and binding, without right of appeal, in all respects, including, without limitation, as to eligibility and/or disqualification of entries.
Participation: To qualify for payment, individuals must have been invited to participate in PurposeMed’s research project, be deemed eligible in the survey, and complete all tasks in Maze.
Limit: One entry per person.
3. PAYMENT:
Payment: Eligible participants will receive a $15 digital gift card through Giftbit, issued by PurposeMed. PurposeMed reserves the right, at its sole discretion, to provide the prize in an alternative form, including cash.
GENERAL: Payment must be accepted as awarded and are not transferable or refundable. No substitutions except at PurposeMed’s option. PurposeMed reserves the right, in its sole and absolute discretion, to substitute the Prizes or a component thereof with a prize of equal or greater value.
4. PAYMENT PROCESS
a. Each eligible participant who successfully completes the survey during the Survey Period will receive a $15 gift card. PurposeMed reserves the right to reject any entry if it deems that the responses submitted by individuals are inappropriate for the Research Project.
b. Eligible participants will be contacted by PurposeMed via email within the first week following the end of each calendar month during the Survey Period. Gift cards will be delivered approximately 21 days after the end of the Survey Period.
c. If a participant: (a) declines the gift card; or (b) fails to accept the gift card or provide PurposeMed with the information required to deliver the gift card in a timely manner, they will forfeit their right to payment. PurposeMed reserves the right, in its sole and absolute discretion, to disqualify such individuals.
5. General Conditions:
a. In consideration of the opportunity to be compensated for completion of the survey, each entrant, by completing the survey, grants to PurposeMed a paid-up, unlimited, irrevocable, non-exclusive, perpetual, royalty-free, sublicensable, transferrable and worldwide license to use its responses to the Research Project (including any feedback) for any purpose.
b. PurposeMed reserves the right, at its sole discretion, to disqualify any individual (and all of his or her entries) who tampers with the entry process.
c. This Prize is subject to all applicable federal, state, provincial and municipal laws and the Prize is void where prohibited by law.
d. PurposeMed reserves the right to cancel, suspend, withdraw or amend this Prize in any way, without prior notice or obligation.
e. PurposeMed accepts no responsibility for loss, damage or claims caused by or resulting from the Prize.
f. PurposeMed will not be liable in the event of an error, technical problem, computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure or any other cause beyond the reasonable control of PurposeMed that interferes with the proper conduct of this Prize as contemplated by these Survey Rules.
g. PurposeMed reserves the right, in its sole and absolute discretion, to require proof of identity and/or eligibility (in a form acceptable to PurposeMed – including, without limitation, government issued photo identification): (i) for the purposes of verifying an individual’s eligibility to participate in this Prize; (ii) for the purposes of verifying the eligibility and/or legitimacy of any Entry entered (or purportedly entered) for the purposes of this Prize; and/or (iii) for any other reason PurposeMed deems necessary, in its sole and absolute discretion, for the purposes of administering this Prize per these Prize Rules. Failure to provide such proof to the satisfaction of PurposeMed in a timely manner may result in disqualification in the sole and absolute discretion of PurposeMed. The sole determinant of the time for the purposes of a valid Entry in this Prize will be the Prize server machine(s).
h. PurposeMed respects your right to privacy. By entering this Prize, each entrant expressly consents to PurposeMed, its agents and/or representatives, storing, sharing and using the personal information submitted with his/her entry only for the purpose of administering the Prize and in accordance with PurposeMed’s Privacy Policy (available at: https://www.purposemed.com/privacy-policy) unless the entrant otherwise agrees to receive further communications from PurposeMed.
