The Survey begins on July 7, 2025, at 9:00 a.m. Eastern Time (“ET”) and ends on August 1, 2025 at 11:59pm, provided it’s not terminated earlier by PurposeMed (the “Survey Period”). Participation in the Survey constitutes your full and unconditional acceptance of, and agreement to be legally bound by, these Survey rules (the “Survey Rules”).
- ELIGIBILITY:
To be eligible, a person must: (a) be a resident of Canada or the United States, (b) have reached the age of majority in their province or territory of residence, and (c) have been contacted by PurposeMed to participate in the Survey.
- HOW TO QUALIFY:
No purchase is necessary. Entries that are incomplete, irregular, have been submitted through incorrect means, or do not conform to or satisfy any condition of these Survey Rules shall be disqualified. Respondents acknowledge and accept the decisions of PurposeMed, or those acting on behalf of PurposeMed, as final and binding, without right of appeal, in all respects, including, without limitation, as to eligibility and/or disqualification of entries.
Entry: To qualify for the Survey, individuals must have been invited to participate in PurposeMed's research Project and completed it.
Limit: One entry per person.
- REWARD:
REWARD: A $30USD digital gift card provided via GiftBit.
GENERAL: Reward must be accepted as offered and are not transferable or refundable. No substitutions except at PurposeMed’s option. PurposeMed reserves the right, in its sole and absolute discretion, to substitute the Reward or a component thereof with a reward of equal or greater value.
- Reward Process:
- Reward: During the last day of the Survey period, PurposeMed will review all responses to ensure eligibility. The reception of a Reward depends on the number of eligible entries received during the Survey Period and, subject to any limits on the number of entries specified in these Survey Rules, the number of entries by the participant. PurposeMed reserves the right to reject any entry if it deems that the responses submitted by individuals are inappropriate for the Survey.
- The qualified respondents will be contacted by PurposeMed sent their Giftbit gift card via email approximately 21 days following the end of the Survey Period.
- If the eligible survey respondent: (a) declines the Reward, (b) does not accept their gift card within the acceptance period outlined on the gift card email (c) fails to provide PurposeMed with any information required to provide the Reward to the eligible winner in a timely manner, then they will be disqualified (and will forfeit all rights to the Reward) and PurposeMed reserves the right, in its sole and absolute discretion to not provide the Reward.
- General Conditions:
- In consideration of the opportunity to enter the Survey, each entrant, by participating in the Survey, grants to PurposeMed a paid-up, unlimited, irrevocable, non-exclusive, perpetual, royalty-free, sublicensable, transferrable and worldwide license to use its responses to the Survey (including any feedback) for any purpose.
- PurposeMed reserves the right, at its sole discretion, to disqualify any individual (and all of his or her entries) who tampers with the entry process.
- If any taxes or levies are applicable to the Reward, it shall be the sole responsibility of the contest winner to fulfill any associated tax obligations. PurposeMed assumes no liability for the winner's tax liabilities, and the winner agrees to indemnify and hold PurposeMed harmless in this regard.
- This Survey Reward is subject to all applicable federal, state, provincial and municipal laws and the Survey Reward is void where prohibited by law.
- By completing the Survey, entrants agree to indemnify and hold PurposeMed harmless from any claims, liabilities, damages, or expenses arising in connection with the Survey. Entrants also release PurposeMed from any and all liability related to their participation in the Survey.
- PurposeMed reserves the right to cancel, suspend, withdraw or amend this Survey in any way, without prior notice or obligation.
- PurposeMed accepts no responsibility for loss, damage or claims caused by or resulting from the Survey Reward.
- PurposeMed will not be liable in the event of an error, technical problem, computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure or any other cause beyond the reasonable control of PurposeMed that interferes with the proper conduct of this Survey as contemplated by these Survey Rules.
- PurposeMed reserves the right, in its sole and absolute discretion, to require proof of identity and/or eligibility (in a form acceptable to PurposeMed – including, without limitation, government issued photo identification): (i) for the purposes of verifying an individual’s eligibility to participate in this Survey; (ii) for the purposes of verifying the eligibility and/or legitimacy of any Entry entered (or purportedly entered) for the purposes of this Survey; and/or (iii) for any other reason PurposeMed deems necessary, in its sole and absolute discretion, for the purposes of administering this Reward per these Survey Rules. Failure to provide such proof to the satisfaction of PurposeMed in a timely manner may result in disqualification in the sole and absolute discretion of PurposeMed. The sole determinant of the time for the purposes of a valid Entry in this Survey will be the Survey server machine(s).
- PurposeMed respects your right to privacy. By participating in this survey, each entrant expressly consents to PurposeMed, its agents and/or representatives, storing, sharing and using the personal information submitted with his/her entry only for the purpose of administering the Survey and in accordance with PurposeMed’s Privacy Policy (available at: https://www.purposemed.com/privacy-policy) unless the entrant otherwise agrees to receive further communications from PurposeMed.
